1. There are people with a disease/curse that makes them prone to accumulating and discharging a large amount of mana/energy leading to explosions. They are called Physis and are treated as monsters.
2. The lady is a physis who caused the explosion that Isaiah(redhead knight) was helping investigate in one of the previous chapters. (The one where FL and Claude run into him holding a baby. And this is probably the same baby). Her unstable powers are being suppressed by those magic circles.
3. The process of explosion is probably painful and living with the stigma of being an unstable monster is probably even more painful. So lady is begging the Duke to free her from this curse aka unalive her.
4. They want to check the baby’s relationship to her but she’s refusing to accept even knowing the baby to spare her from being ostracized for being related to a monster😭
5. The Duchess was probably a physis too. She left them to protect the Duke and her kids from her unstable powers and to save them from the stigma…(alluded, not confirmed. There may be other reasons too)

Just looking at the prison guard’s attitude and the magician’s surprise on learning the Duke treats the ‘physis’ as humans you can guess what her life will be like in future if it got out that she’s related to one.
