I think if you miscarry late term (third trimester) you still have to give birth to the baby. Your body can’t destroy the matter that makes up the baby’s body so it has to expel it through birth. The baby’s body is probably the reason the empress got sick and definitely why she was bleeding since it was in the process of expelling the body.
