I think the person who took over the Duke is Elise’s boyfriend from the other world. I know it sounds messed up what he did but if anyone recalls in the chapter about her last day in her previous life, when she asked him if he was going to kill her, he said he was but he was joining her. I think he thought it was best for her to die as she was instead of slowly wasting away (depending on what she had) and that he did kill himself shortly after. The Duke’s personality completely changed and instead of being cold, he looks at her very lovingly. In this chapter, Elise acknowledges that someone else is inside the Duke so my guess is it’s the former boyfriend.
