I’m pretty sure they aren’t actually related, he’s the successor to her maternal family line which is probably why she said they were related but he was taken in from a branch family.
It’s normal in the ancient time to have relationships with each others. Even very close cousin are married so i won’t be surprise if they did get married. BTW, Gunwoo is the male lead (the cute blush guy) 🤷♀️
