It’s described in the novel that first there was pain, but eventually she grew numb to it, then she felt pleasure and felt revulsion that her own body could betray her. Even when she’s given an option to run away she doesn’t, she’s just done. So when she’s given a poison to kill the “ML”, she doesn’t use it on him. She uses it on herself and instead of using it in the way that causes instant death. Ana does it so she dies slowly. Because that will hurt him more. So he takes her to the frigid north, where she hates it, nearly dying on the way due to her weakened state, the cold and constant moving.

There Ana lives out the rest of her life in relative comfort, never moving from her bed except for walks to the green house. She never laughs, crys or smiles. The maids think she’s a bit creepy, they aren’t aware of what happened since all the ones who knew anything were replaced. So they think the “ML” is really in love with Ana and that she’s a bit ungrateful. Ana doesn’t care anymore, and she slowly fades away. On her deathbed, she makes Theo promise that if there is a next life. That he’d stay the hell away from her and that she never wants to see him again. Then makes him promise to stay alive after she dies, that would be his atonement. To live forever alone and wallowing in the misery caused by his own choices. She makes him go get her some wine and dies by the time she gets back. She didn’t want his face being the last thing she saw before she died.

She’s buried in the greenhouse where she loved to walk, and he lives to old age. He never remarried, never had children, never went back to the capital and lived a quiet life. When he died, he was buried in the green house in the furthest corner away from her, because he was ashamed his body was old and ugly.
