I think in this kind of manhwa medicine is based on eastern traditional medicine or TCM. In TCM there is a way to diagnose someone’s illness through a method called pulse diagnosis. You can google it for details. Honestly I don’t know if there’re words for people who practice this, but if they are really good at it, they can really tell various things just from doing that. I know because I have both consulted for my condition from hospital doctors and TCM physician, and the latter can give me similar diagnosis with me talking similarly like when you consult to the former. Yes, it’s obviously an “energetic” (esoteric) pulse diagnostic like it’s still in use in Indian Aryuveda or TCM or Unani – the latter goes back to at least ancient Greek around 500 BC and actually up until medieval times.
Differently from how it is shown in Victorian fantasy manhwas though, this was never an accurate and reliable diagnosis technique in Europe. However, also treatments back then were also sometimes really absurd (opium called “Laudanum” was widely used as a painkiller, even for babies, and poisonous medicine was common), hygiene regulations for medical workers were non-existant, life expectancy was low and infant mortality high.
