My question is, Why doesn’t she just tell him it makes her unhappy? Because she’s just constantly made out she’s okay with it and he’s not a mind reader.
Maybe he’d immediately end the marriage with her leaving with the nothing shes brought to the marriage, but maybe he would try and work out a compromise if he knew. Like, give her some money so she can start a new life. Or realise she’s the most important and give up seeing others.

Well, to answer your question without assuming info the story hasn’t given us, I think she still has feelings for him or she wouldn’t be so sad when she sees him with other women. I also think almost anyone would choose the life of a well fed noble over the life of a starving peasant.
If I assume the story follows some history then there is no divorce for her unless she is accused of infertility. He lied about how he felt about her to get her to marry him in the first place but I think she would have chose him even if she knew he wasn’t faithful bc she was in a desperate situation
